Requests for Recommendations

 

Under the most recent FERPA interpretation guidelines, it is now strongly recommended that written permission is obtained from students who are requesting phone or written references for employment or graduate schools purposes.

If you would like to request a recommendation from me please do the following: 

1. Complete, Sign, and Return the appropriate form to me either in person or via email

• Letter of Recommendation

• Phone Recommendation

2. Provide me with the following information

• Description of (and/or web link to) the program/job/scholarship you are requesting a letter for

• Information on how to submit the letter of recommendation (i.e. mailing address, weblink, etc.)

• A short resume

• A description of your short term and long term goals
